While Legend of Zagor has connections with Titan, it is set on a different world, Amarillia.

Not just technically but pedantically you didn't specify only game-books and therefore Stellar Adventures (Sci-fi FF RPG) is somehow set in the FF universe but not on Titan. I seem to recall a debate elsewhere Phantoms Of Fear is set on an alternate planet; I don't remember whether the evidence Phantoms Of Fear isn't set on Titan is because there is no reference to Titan with the exception of the otherworldly feel and the reference where you seemingly dream of being on somewhere that is Titan, Earth (or neither, or just your own planet in the future) in present-day, that is probably convincing evidence and no evidence at the same time. While Titannica is definitely not the most trustworthy source it does note several references to other beings and things on Titan which leads me personally to believe Phantoms Of Fear firmly set on Titan, counter-suggestions such as "it was a dream" or "magic enables travel between planets" seeming more unconvincing to me. I would say it's crystal-clear Phantoms Of Fear is set on Titan but the dream reference might-or-might-not be set on Titan, of course just having the odd reference that is not set on Titan would qualify Mage Hunter and Beneath Nightmare Castle among others but that would be like saying a banana split is watery because you ordered a glass of water with it.

I seem to recall a debate elsewhere Phantoms Of Fear is set on an alternate planet Madness. Your character in Phantoms lives in the same forest that the hero of Masks of Mayhem can travel through, and may meet up with a former adventurer who retired after getting lost in the Maze of Zagor.

Talisman Of Death is set on Orb rather than Titan. Sylas included Talisman in his list of FF books not set on Titan. Some deluded fans have tried to suggest that the hero of Talisman was transported to Orb from Titan, but that's clearly nonsense, as the book clearly states that 'you' come from Earth - it gets name-checked three times in the Background, and again in section 400.

Phantoms of Fear makes repeated references to Affen Forest, Lake Nekros, Khul and Titan throughout the book.
Even 400 begins Now that you have rid Titan of Ishtra...

Having never owned or read the Titan manual, I do have some questions here. Does titan detail what each region or continent and what it is like, or is it just a series of maps?
I only ask because each author, particularly later in the series, has created areas that are quite unique and belonging only to that author. Eg, did Keith Martin come along and say "I'm going to write a vampire book and set it here (choosing a spot on the map) or does titan already set up an area equivalent to Transylvania for him to use?
Also, where exactly is Scorpion Swamp? It always felt pretty anonymous location wise

Maps only make up a small portion of Titan - the Fighting Fantasy World, but the amount of detail it goes into about different regions varies. The book is more of an encyclopaedia than an atlas - there are sections on geography, history, people, species, calendars... Some of the settings of later books are already in there, while other regions were left vague and filled in later. To give a few examples, Vymorna is covered in great detail, Sardath and Ashkyos are on the maps, as is Crucible Island, but there's no sign of Mortvania or Lupravia anywhere in Mauristatia.
Scorpion Swamp is in western Khul, some way to the north of Neuburg.

Makes sense Titan being detailed enough for someone to build from or vague enough that an original creation can be shoe horned in. Mind you, like other views I've read, not sure shoe horning Crystal of Storms to Titan fits right, surely they could have allowed that to exist outside
